Payment can be made via check or credit card.

Note that Insurance is the patient’s responsiblility. Please verify that your insurance plan covers services provided by our facilities. Our facilities cannot be held responsible if a claim is denied because it does not meet specific requirements. Patients who do not have full insurance coverage are required to make payment at the time of service. Co-payments are due at the time of service.


The Health Insurance Portability & Accountability Act of 1996 (“HIPAA”) is a federal program that requires that all medical records and other individually identifiable health information used or disclosed by us in any form, whether electronically, on paper, or orally, are kept properly confidential.
